Ward-led Enablement team helps customer regain mobility during hospital stay
Mark was admitted to hospital with restricted mobility and difficulty managing his medication. He received help from the ECL Ward-led Enablement team during his stay and upon discharge, received support at home to further aid his recovery.

As a result of an injury to his leg, Mark had difficulties in walking and performing daily tasks, and was admitted to hospital for treatment.
With help from ECL Ward-led Enablement team, he was able to swiftly return home and receive reablement care in the comfort of his home.
The team worked with him while on the ward to improve his walking through the use of a walking aid, as well as supporting him to undertake daily exercises to improve his stability and mobility.
He said: “I would have been lost without ECL’s support during my stay at the hospital.
“There was nothing I disliked about the care; everyone did a professional job – I don’t know what I would have done without it.”
Once Mark was discharged from hospital, the ECL team continued to support his recovery, helping to improve his overall wellbeing and ensure that he could stand and move around his home and perform his daily tasks.
The ECL Reablement team worked with Mark to create multiple goals which they then created his care plan around. Together they worked towards achieving them during his six week home care period.
His primary aim was to get into a routine with his medication. ECL Community Care Assistants helped him set a routine and showed him techniques to help him remember when to take it. They also introduced him to using a daily pill box with sections for each day related to the time the tablets needed to be taken.
His second goal was to improve mobility around his home within two weeks. He made good progress by navigating into the kitchen with the help of a frame, occasionally even moving around without it.
Mark also wanted to regain the ability to shower independently within a two-week period. He made great strides towards this goal, and within two weeks could sit in the shower to wash on his own.
His final goal was to prepare a small meal for himself within two weeks which he achieved too. Initially he was only able to handle ready meals and prepare simple dishes due to the length of time required to stand but as his care progressed he was able to stand for longer and prepare a wider variety of food for himself.
Despite facing challenges with mobility due to his leg, Mark remains hopeful about his progress.
I am pleased with what I have achieved so far. I would like to get out more but I'm currently not capable of because of my leg. I cannot fault the help. The service overall is exceptionally good.
